A dense brunescent lens with poor dilation. Because of the poor dilation, when the surgeon spun it, they put too much pressure posteriorly. This probably would have been OK in a normally dilated pupil because the A-P force used when imparted peripherally is less strong because the lens is less dense out there. Centrally however, the force is greatest because it overlies the densist part of the nucleus.
